Awards Records Info


If you need information about an award you believe you or a family member was due or if you would like to know more about the circumstances in which a deceased family member received an award, you will need to send Standard Form 180 or a letter containing all the appropriate information to the Army Reserve Personnel Center.

Standard Form 180 is available from the National Archives and Records Administration. Follow their instructions for downloading, printing, and mailing the form.
